The rebate form asks "Zip Code of Retailer Where Purchased" ... how do I answer that for online retailers like Amazon? Do I look up Amazon headquarter's zip code or ?

The limit is 1 per person. "Limit of one (1) Qualifying Purchase per receipt/entry, and limit of one (1) receipt/entry per person during the entire Promotion Period.
