Looks like it is not preactivated. No info on activation deadline, but SIM does not expire... So possible service does not as well, but don't hold me to that

Check if your phone is allowed on att network boost will say it's allowed
Att says not allowed and you will not get data or phone calls ringing up customer service will not help they will not tell you
Why phone is not working you will be on the phone for hours with them

Yes, you would need to create an account and add a credit card. Use a disposable (virtual) one if you're not planning to port out. There are reports that Boost tried to charge for another month even when the line is cancelled and not ported out.
