When you added that medium pizza to your cart, it counted toward the $20 minimum, so if you added 2 medium pizzas, the total of the food would be $10. Now, after the changes, you would need to hit the minimum of $20 not including the free item - so you would need to add 2 medium pizzas to hit $20, and then you would get a third pizza free.
Keep in mind that DD charges anywhere from 15%-30% for sellers depending on their merchant status, so most restaurants increase their prices by 30% or more compared to their in-house prices. Sometimes you can select pickup to get their in-house prices, it depends on the merchant. If you want the best deal, make sure they have cheaper prices when you select pickup and go that route instead.
