I use a 20# vest for almost everything from pullups to hiking, it's a terrific way to make things harder without having to just go for more speed or more reps.
I'm not sure I'd even notice 10# but it's all relative to your bodyweight and fitness. The smaller ones fit a lot better, too, less flopping around.
Weighted vests are great for exercise. Unlike normal exercise, you don't do any extra activity. You just wear the vest and do what you would do normally - include going for walks. The fact that you are carrying extra weight means your body has to work harder - like if you carried a gallon jug of milk in each hand on the walk. Benefit over carrying weights are that your hands are free and it is a lot easier to carry weight on your torso than on your hands.

Is this good for someone with bad lower back or do you think it will make it worse?
Always best to discuss with your doctor if you have an injury. In the case of a back injury, best thing I can think of is to lose weight. Extra weight indicates a likely inflammatory state of the body. Inflammation will aggravate everything including back pain.
