modern cars are fuel injected so not anything new (edit: except for throttle body cleaning, which it says on the can). This is more for fixing lawn mowers, leaf blowers, weedeaters, etc that all still have carburetors (which are less efficient, but simpler since they typically don't require a fuel pump, fuel pressure regulator etc)

Your throttle body is still on modern cars. It is the big round opening past the mass flow sensor (you can't see it, but it is the final step where air enters the engine block itself). So you have your air intake, then your air filter housing, then right after that you have the mass flow sensor, and then, the throttle body. This directly can affect the smoothness if your idle if you have a lot of carbon buildup over the years. So say, 10+ years your idle can just get worse. Some percent of that reason the idle is worse is throttle body. It's minimal, but it does matter.

So, you use this cleaner. Some people make the mistake of using Brake cleaner, but many throttle bodies have special coatings on the steel and will be ruined by certain solvents. Like, don't make the mistake of using the wonderful solvent that is diesel fuel to clean this. Get a brass brush and throttle body cleaner.

This is a non-issue for cars probably less than 10 years old though. This is for people that push their cars past 150,000+ miles and looking to really clean them up. So, it does still matter with fuel-injection vehicles, but if you are 50k miles and a garage is asking you to do this, they are just ripping you off lol. This makes almost no difference unless you got a couple mm of caking on carbon affecting the flow where you might even need to reprogram your idle.
