Same thing happened to me. Chat told me there was a mismatch between the name on my shipping address, and my credit card. I've placed dozens of orders previously without issue.
They couldn't re-instate the coupon, nor could they unlock the discounts again on the website.
In store, generally, item limits on coupons are enforced on a per transaction by the system, and any items above the limit in that transaction are not discounted. However, the system does not enforce item limits across transactions. A cashier/supervisor could refuse to process the sale if they see you exceeding the limits.
