Here's a little Wiki I created for earlier deals with tip and instructions for the rebate.

REBATE TIPS AND REQUIREMENTS
Many don't get receipts or proper receipts sent with their order. Their website does not generate a good invoice. Call to make sure you get a proper receipt that will meet the rebate terms. They can email it immediately. Email support is usually fine, but calling is better.

The invoice MUST have:

• Order Date
• Items must be listed separately
• Retailers name on receipt

Adorama ships the printer and camera and other stuff separately. They do not always include all the receipts. In general you will get one for each shipment but sometimes not. If you don't see one for the printer open the box, they had slipped one inside on one of my previous orders. Another order I just did not get one.

It is fine to use two receipts as long as they are both for the same order bought on the same day. Adorama can generate a very basic combined receipt, that lists all the items separately. Some of the reps don't seem to be able to do it and will just get you the two separate receipts for each shipment.

I have submitted the rebates both with the two receipts and with the combined recept and have been always approved.

Other Requirements & Tips
You need to have the products in hand to submit the rebate as you need the serial number form the printer.

You need two UPCs to claim the rebate. DO NOT peel the top layer with the UPCs, cut them out leaving a hole in the box with all layers of cardboard intact.

Found on the outside of the printer above the serial number
UPC from camera box(Not the Premium Kit Box)
Use the UPC from the camera box which is inside the Kit box, do not use the kit box UPC. Do not use the secondary UPCs that are inside top flaps of the box. Use the one on the outside below the picture of the camera.

Paper UPC is not needed for the rebate

You have two ways you can file the rebate.

1. Download the rebate form. Follow the directions, fill it out and attach the other required documents and mail it in.

2. You have the option to Pre-submit the rebate online and print it for mailing with your required documentation. Go to http://rebates.usa.canon.com and click on the "Pre-submit your Rebate Claim" button to pre-submit your claim.

If you use presubmit option you do not need to use or mail in the form linked in the OP. Follow the direction on their web page and a form will be generated at the end. You will submit that form with your other stuff. You still have to mail in the form that is created and your supporting docs to get the rebate.

If presubmitting the Offer Number is found in the address that you would mail the linked paper rebate to.
The Primary Product is the Printer. The Pro-100
The serial number is right below the UPC on the outside of the box, no need to open the box.
The receipt must show the retailers name, order date and have the products listed separately with the date of purchase and the purchase price circled.
Do not peel the top layer with the UPCs off of the cardboard. They want a hole cut in the box and all layers of cardboard intact on the UPC.

CAREFUL WITH THIS
Depending on what bundle you buy the paper is different. Make sure you pick the correct promotion for the paper with your order. The paper will be either LU-101 or SG-201. Make sure to pick the right offer from the list if presubmitting or from the check box on the forms.

If presubmitting after filling in the offer #, primary product and serial number carefully scroll down to you find the correct offer.

KEEP COPIES OF EVERTHING JUST TO BE SAFE, IT"S A BIG REBATE.

Basically you may need pictures of the serial numbers from the camera and printer in the event your UPC get lost at some point in the process. Copies of the UPC are apparantly not sufficient. You may want to take pictures if you plan on selling the items before getting the rebate.
