No just the IMEI for the physical SIM - the one that shows up in "My Phone" section of your TMobile account. Once that is unlocked, the ESIM is also unlocked automatically.

You will need to upload photos of your Costco receipt and imei from the box. Without those, your rebate will be denied, which might further complicate things. Better to wait.

Just factory reset the phone so it's at the setup screen. The IMEI and serial are available there and that's all they need for the trade.
