The rebate form I printed clearly says cut UPC label from shipping box or product box and stick it on the form. Form even has a box marked to stick the UPC label.

This rebate does not require UPC cutout. Please submit your signed rebate form and Dell Invoice. You can always double check the required list of documents on the OP and on the rebate form section #4.
