If they are all on one receipt/packing slip, you need to include one rebate form, 3 original upc labels from boxes, and one original receipt or packing slip.

SAVE20 is still valid for PST because technically it's not midnight yet. the rebate form is valid april 25 which is not technically the 25th in the PST zone, if that makes sense.
