I have called in a few rebates. You give them your information, and they send an email with the form attached (or a link to the form, I forget) for you to mail in with the invoice and UPCs.

they've always been debit cards. You're supposed to slide as credit, but some terminals recognize it as only a debit card and skip the debit/credit question.
