Seems like only some grandfathered plans are eligible. My Simple Choice plan isn't eligible apparently after chatting on FB with T-Mobile. 😔

You want to do it in Costco only. If you do at any T-Mobile store, again your credits will come as monthly credits. So this deal works strictly in store at a Costco only.
