Once you fill out the form, it should come right away. I'd suggest to check your spam or fill out a new rebate form. Please make sure you are putting the right e-mail address too.

I emailed SlickDeals and received an email reply on the 9th. They said the rebate started on the 5th and ended on the 5th, yet the rebate form said the 5th-8th.
