The rebate form states: Limit 1 Claim per eligible product per household. Might be out of luck since you're using the same invoice address for both rebates :/

Yes you need to cut out the UPC from the box including the cardboard behind it. IT is detailed very well on the rebate form

No, it's a MasterCard rewards card now and that option in not available. Also note also that after six months a monthly fee will be deducted. Use the card up quickly.
