I usually enter my phone number linked to my account. When you get to the pump, press ALT. Then FUEL (or FR) REWARDS. You will then see the option to swipe the card or enter your phone number.

I tried the code that I claimed on December 16 today and ATOM said it was expired or it had exceeded the number of redemptions already. Have a ticket in to Fuel Rewards customer support.
