No, this is a BYOD (Bring your own device) promotion, so it only works when you bring in a phone that you bought elsewhere and then activate it onto a new T-Mobile line at Costco.

Yes, if you "Bring all Your 4 Devices" and Activate and pay bills for 3 bill, then you will definitely get $150*4. As far as i know, per account, you can add 12 lines. I may be wrong on this.
