To apply for a credit card by phone, please call 1-800-932-2775 and a personal service representative will help you. Please mention priority code VABJOM. It took me 14 minutes to apply.
This is through Bank of America. I already have the Visa Signature Cashback Card and asked if I could apply for this one and the rep said it's a different promotion, so it was OK to apply.
Not sure if this number will help if you are not instantly approved... but this is the (Credit Analyst) for reconsideration phone number 866-458-8805 (only open during normal business hours).
*Update 7/23/2012 for those that already have this card* - I already have this card and called in to apply for it anyways. I told the rep I already have the card and she said I could still apply and she was right. I just now called the reconsideration line and the guy approved me for this card and the BOA Mastercard giving $300 cashback for a total of $700. Good to keep in mind in case you find a great deal on a card you already have... just call in and see if you can get another card with the great promotion.

Edit: Applied over phone and got approved. The CS person was able to confirm the code VABJOM was for the $400 cash back after spending $500 in the first 90 days AFTER I finished my application and approval over the phone. This promo also includes 0% APR for first 15 statement cycles.
I'm still going to call once I receive the card to confirm the cashback bonus of $400 after spending a minimum of $500 in first 90 days.

Try calling and applying for this card. I have the BOA cash reward as well and they said it's OK to apply for this card since it's a different offer. Worth a shot for an easy $400.
With that being said, I didn't get an instant decision but that may be because I've applied for lots of credit card offers in the last 12 months... or it might be because I already have this card and the lady gave me bad info. I'll have to wait and see.

I applied for 3 cards in the past 2 weeks not sure if I want another hit on my credit report but an 80% spend bonus is kind of unheard of these days...
