Had the same problem. They escalated it to a manager and said it'd be looked at within 6 days. A few hours later, my rebate was approved. YMMV though.

Well there is no way to prove scanner error or purposed fraud so.... I understand dropping a paper out of hundreds or maybe thousands a day, but that is entirely different than removing an invoice...

No but if it was that wouldn't keep them from scanning it. It's probably just a mistake on the part of the scanner operator. They don't have a history of losing parts of submissions.
