At least you got approved. Stupid TMO rejected my rebates. I was on phone for two hours with their Rebates team and she offered partial approvals to me. I said, NOGO. I ended up writing to John Negre.

You can only submit rebates online. Costco will print you trade in labels to send your phones (via USPS) to Assurant, the company which manages trade-ins for Tmobile.

He shouldn't need to fill out the rebate form to get the $25 cash card if he ordered online. I received my cash card with the accessory when it came in the mail.
