begins at 3:00 p.m. Eastern Time (ET) on August 2, 2018 and ends at 10:00 a.m. Eastern Time (ET) on September 13, 2018
Limit to one (1) Sweepstakes Entry per valid e-mail address
open only to all legal residents of Canada or the United States (excluding the states of Florida and New York, all U.S. territories and possessions, and all overseas military installations), who are sixteen (16) years of age or older
One (1) Grand Prize with an approximate retail value ("ARV") of $1,000 CAD / $800 US:
• A 30-minute meeting with a CCM endorsee to be held no later than November 16,
2018 in Canada or in the United States, in a city near the Grand Prize winner's
residence; and
• Two (2) tickets for a hockey game of such CCM endorsee occurring on or about
the same date of the meeting referred to above.
One thousand (1,000) Secondary Prizes with total approximate aggregate retail value
("ARV") of $46,190 CAD / $41,690 US:
• Ten (10) pairs of JetSpeed FT1 Skates (ARV of each: $999.99 CAD /
$849.99 US);
• Ten (10) JetSpeed Sticks (ARV of each: $299.99 CAD / $249.99 US);
• Thirty (30) JetSpeed Gloves (ARV of each: $219.99 CAD / $199.99 US); and
• Nine hundred and fifty (950) JetSpeed Hats (ARV of each: $27.99 CAD /
$25.99 US).
Total ARV of all Prizes: $47,190 CAD / $42,490 US.

I won the hat,but the declaration form they want returned in 5 days states I have to reside in Canada???
Did anyone else that won a hat recieve that email from them?

They did reply to my email and said...Simply replace the Canada aspects of the form with US ones. EG: Replace Province with State, postal code with zip code. and congrats on win ask if any more questions happy to help blah blah blah
I have no clue how to use any program on a PC where I can fill it out and sign it to email it back.
