Eligible Console: Valid toward the purchase of a (New) Xbox One X Console w/ State of Decay 2; SKU 155211. (New) meaning towards a new console.
What is needed when you trade? Basically everything that originally came w/ your console (ex: Console, Controllers, HDMI/Cables, Power Brick etc; anything excluded will be deducted from the trade value). An ID/Driver's License and/or thumb print (depending on trading laws in your state); some might go as far as taking a photo.

Correct, it has to be towards the purchase of a New Xbox One X Console; specifically the New Xbox One X Console w/ State of Decay 2.
There is slight chance you may apply it to another version console if they don't have it in stock but that would be upon a manager approval etc (YMMV).

You can try but typically it does not. I tried with the last promo and their system gives me the normal trade in value on the second system when scanned in.

Most likely the best you might be able to do is resell it. I'm not sure if you returned it if you'd get a full refund or refund minus the difference of what a normal trade in would be. Imagine it has to be the latter otherwise people would use GameStop to "sell" all their used systems.
