2. How to Redeem Your Offer

The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

my local costco had iphone 11 64gb for 630$.I got the device and activated one line at 60$ with autopay per month.i traded iphone 8 which got trade in value of 145$ and rest 580-145 will come via mastercard.
the tradein credit will be applied after 1st billing cycle.
its a nice deal only if you are getting iphone 11/XR.if you have iphone 6,7,you will get more in mastercard.
what are the approximate taxes for the $30 plan? is the $30 plan actually cheaper than the $35 plan?

The reason I ask is the Costco rep says he doesn't need any trade in details now - and asking me to do that online

This is nothing new and has repeated with past T-Mobile promotions. The reps give you a fixed value based on the device and the rest through the card.
