I'm pretty this rebate submission is the similar, so don't put in your trade-in's IMEI, it should be your new iPhone 11's IMEI when do the submission at their Promotions website. I had to submit a copy of Costco receipt and a picture of the back of the new iPhone 11 box, showing the IMEI

Per Costco booklet, there is no activation fee, but it is Tmobile policy or Costco will reimbursement. Anyway, I guess you don't need to worry about it.

Monthly charge:
I signed up Autopay. I remember $70/month price requires that.

Port in:
For my case (H2O), I called H2O customer service: 800-643-4926 and verified my recent 3 call/answer numbers. They then messaged me my H2O account number and told me the PIN over the phone. It turns out that's all you need for port in. BTW, even though I left the store for a few hours, my old phone still work at H2O so the port-in may need a whole night to complete.

Trade in:
They print a shipping label (no box or envelope supply) and you are supposed to ship your old phone within 30 days. You can NOT do the trade in at local T-Mobile stores.
For my case, they entered the IMEI number and estimate it cost $62 (sorry, forgot exact numbers, but less than $100 for sure). The associate told me that that $62 will come in as statement credit while the rest ($580-$62) will be sent via gift card.

BTW, judging from the fact that it is a flat $580 regardless of what phone you trade in, isn't it stupid to trade any phone other than the lowest tier, namely 6s? Any phone better than that, you won't get extra value. Maybe this deal is exactly to allure the shabby SDers...

Get the promotion
I was told by the associate that I need to wait 90 days before I can cancel the line and switch carrier (I assume by then I will also get both credit and GC). Anything earlier than that will either forfeit my promotion or, if I get the GC/credit, they will bill it back. Also, that's the timeline that Costco will be paid by carrier for the transaction. For safe, I may be willing to hold 3 months before I change anything. If so, here is all the money goes:
1. Monthly charge: ($70-$30)*3=$120 , $70 is T-mobile price, $30 is H2O price
2. Phone cost: $630-$580=$50 (I bought at NH, so no sales tax)
3. Trade in: iPhone 6s.
So essentially, you spend $170 and trade in a iPhone 6s. After 3 months, you get an iPhone 11.

If you add one more line ($50/month for Magenta), the cost will look like:
($70+$50-2*$30)*3+(630-580)*2=$280
Or say, if you trade another iPhone 6s, the 2nd iPhone 11 will cost you net $110 (=$280-$170). Surely the more you trade, the better average you get. I think the line limit is 4/account.

That is for bringing your own device. You cannot get a new phone with those lines until you get your rebate in 12-14 weeks.

H2O5GB unlimited is $30/month with autopay. That's simply. (https://www.h2owirelessnow.com/plan/monthly)

What ($70-$30) means that every month, I don't have to pay $30 at H2O but I will have to pay $70. So the difference is $70-$30 from the status quo.

Currently having issues with adding a 9th line to my T-Mobile ONE account. Costco rep could not add a line through the Wireless Advocates portal. We then proceeded to call the T-Mobile Customer Center line and escalated the issue to a supervisor. The supervisor claimed that they were experiencing an internal system issue where T-Mobile ONE accounts only support up to 8 voice lines even though it clearly states that the maximum number of voice lines is 12 on their website and that they could not do anything on their end to add the line. So I left Costco without the ability to add a 9th line to my account...

Has anyone faced this issue and found a solution to it?

This is a long shot, but I am throwing this out there if it comes to this point -- Will this promotion work if I somehow manage to add a line at a T-Mobile retail store (or even if I manually add the line on my account through my.t-mobile.com), then go back to Costco to purchase the phone and initiate the Trade in and Rebate process?

The rep could have messed up, got into same issue. Called up Tmobile promotions and they were unable to send the return labels so I had to call member wireless and they did paper work once again and sent me the RMA and RETURN labels
