2. How to Redeem Your Offer

The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

I have one iphone 6s plus and looking to make 4 lines to max out
I figured out the problem. The costco rep put an entirely different IMEI on the promotion sheet. So I put the IMEI from my iPhone box and it worked. Submitted pics of the box and receipt and successfully submitted. Hopefully the gift card arrives in 6 weeks.
So yes you have to individually submit the info on the t-mobile.com/promotions page. Costco rep doesn't do that.

Once they enter that info, they print out a sheet with a label on it. You mail it out on your own
BTW I have a Tmo One plan.
But looking at the wiki, it doesn't seem they allow talk and text plan only.
