The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.

Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

Though it seems the issues come about from people not following instructions and/or trying to game the system such as sending in a phone with screen damage when the info clearly says you can't do that.

Once unlocked can these phones be used on a cdma network, like for pageplus?
