The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

I've got a set of instructions but too many codes to enter. I just shipped the 6s with the provided label today.

I figured the imei is the main thing, I was asking for a solution to the error of anyone knew that's all.
You can request kindly that you'd like to enter in their system yourself for security reasons. They don't need to know anything. The credit system will automatically check your name, address, SSN, etc to verify your credit rating. If your tell them the incorrect SSN, you will not qualify as it will come out as invalid.
If you are not comfortable working with that agent, go to another Costco of come and deal with a different agent.
Just did the deal in Costco, and the total price after 6S trade-in and 3 months w/ T-Mobile will come out to an effective cost of $219 for the phone. Truly amazing.
I will say though when I asked the Kiosk agent to confirm that I can cancel in 3 months if I want to, he said it was 6 months and anything before 6 months T-Mobile can try and get money back. I asked him to confirm this, and where that was in writing and he said it's not in writing nor is the 3 months. He then tried to tell me to go to another store because I was going to cancel the service, and that I was "stealing" from his company. Politely told him that was nonsense and the deal terms are the deal terms, and he eventually proceeded forward.
So fair warning, apparently some of the folks working there are against this offer and might give you some grief.

I paid in full, walked out with a phone, the guy said I will get the full $580. And the email I received from T-Mobile said my 6S trade in was valued at $31 lol.

Now I managed to get into the form but it seems that I have to wait until I port may definitive number for this to work properly as they need to sen a text about the promo.. Is that right? - The new line I got from Costco is not the number I'll be using from thursday ( when I port my ATT number) .. Does that make sense?
