You will lose both the phone and the rebate and it will be a hassle to get T-Mo to pay since Assurant will notate it as broken phone and TMO will not help with that note.. Get the screen replaced(should be $40) before you mail it out.

if you use a priority box, you will have to pay priority shipping.
The label is for a first class package. So try to find a box that doesn't weigh as much. The limit for first class is 13 oz.
I just use boxes from amazon that I have laying around.

Did anyone tried with T-mobile advantage corporate reward offer along with this trade in offer? Is there any chance they can reject $580 offer for clubing with $25 offer?

1, Reward card submission must be submitted online or postmarked within 30 days of the new activation.
2, Eligibility is restricted to new Advantage activations with device purchase on a qualifying rate plan.
3, Your account must be in good standing at the time of account validation.
4, Reward cards may not be combined or used with any other reward card offer, free, or other promotional offers.
5, Reward card may be redeemed at participating T-Mobile retail locations through the expiration shown on the card.
6, To be eligible for the reward card: (i) your submitted materials must be validated by T-Mobile and (ii) you must have an active account.
7, Reward cards will be mailed to your current T-Mobile billing address. Valid reward card requests take up to eight weeks from receipt to process.
8, Do not send reward card request form or reward card with your T-Mobile bill for processing. If you would like to apply the card to your T-Mobile invoice, please take the card to a T-Mobile retail store.

can some one please tell me, if this deal would work only if costco has stock in hand.
when i spoke to costco yesterday, he said they have not been doing T mobile and he could not check it online sine Tmobile server was down. to check back today and he can do it online only to ship the phone to my house. is this something any one has dealt with?. i dont want to be struck with other deal and get into monthly credits instead.

just go to a mobile repair shop and get the screen replaced for like $30!
