I owned a 12", it was hard as a rock. You can read reviews of the 12" zinus or others on amazon

Layers and whatnot can matter, but this is the general rule of thumb.
It really doesn't. It has to do with the density of the foam (mostly) and the temperature (quite a bit less).
5-6lb foam (tempur-pedic, zenus) are firmer and zenus is especially firm when the mattress is cold. I have a zenus I bought for my parents guest room when I come visit.
3lb foam is my favorite (bed in a box), it's soft, but not too soft. I've slept on this as my main bed for 12 years.
Without knowing the density and construction, you really can't infer anything about hardness from the thickness.
And hybrids (Springs with a foam cover) seem like a bad idea.
