NOTE: if one is planning to buy any Xbox console via Xbox All-Access, then keep in mind that one is only allowed to stack up to 36 months of Game Pass Ultimate. All-Access is essentially a 2-year contract which covers the cost of an Xbox console and 24 months of Game Pass Ultimate, just as a cell phone and the service for it works. After 24 months of All-Access, the console is paid-in-full (one now owns outright) and one just needs to pay for any service subscription (Xbox Live, Game Pass, etc). If I'm not mistaken, when doing the All-Access package, 24 months of Game Pass Ultimate is immediately added to one's account, which in turn would chop off any months in excess of 12 months of Game Pass Ultimate that is on one's account. If the contract is cancelled early, then the remaining unused months are removed from your account and the console will need to be returned to Microsoft. I happened to call Microsoft and ask them about various scenarios and that is what I understood from the call, since I still had 25 months on my account at the time of the call. Terms and conditions for All-Access may have changed since my call. YMMV

The only difference is it's not streaming. You download the game and can play it online or offline. I don't have PS Now but I do have Ultimate from the $1 deal and I have to say it's been worth it. They've had games like GTA V, Nier, RDR 2, Kingdom Hearts 3, Final Fantasy XV, The Witcher 3 etc. The assortment of Xbox, Xbox 360 and XBox One games is pretty amazing in quality so far.

It also includes Games w/Gold, which every month, gives you two games to keep forever (a backward compatible Xbox 360 game) and two Xbox One games, which you can keep in your library for as long as you have a gold subscription (simiilar to PS+)
