Once you sign-up online using the instructions provided, a membership number is populated. When you go into a store, you can provide any piece of information (e.g. phone number or email) & Sam's Club Store will print out your membership card for you.

You probably have to actually purchase online at samsclub.com. Just order something for pickup at your local club to avoid shipping charges. That should satisfy the requirement for the second gift card.
