Was the sim card free or were there any service fees for doing it in person at Costco?

If you do it through T-Mobile there is a sim card/service fee attached to it.

No cost or anything at Costco. T-mo direct will charge $30 but most have been able to get that refunded

Can anyone confirm what are the maximum number of lines that can be ported in to take advantage of this offer .. I wanted to port 6 lines from mint.

For new accounts, if you are switching from another carrier, there are better deals at T-mobile (online and in-store).

Depending on which phones you currently own, you can get good deals on trade-in and getting new phones.

$100 port-in promotion will stack with the carrier switch over deals.

Please visit a T-mobile store to find out what's the best deal you can get for the 5 lines (plan + phones)

They needed an imei and sim number for me. Luckily they were on the box for my A71 i just got through samsung. Used a picture and read off numbers

Yes, the rebate control number is specific to the person/transaction. It gets printed with the rebate redemption form.

The rebate redemption form states that the Control number cannot be duplicated.

The promo code is same for all, but the control number is unique.

The reps should be adding a new line, BYOD IMEI, etc, and the system generates and prints the rebate redemption form. The rep gave the printed form which had all the information - promo number, control number, purchase date, IMEI number of BYOD device, activated permanent phone number, and Kiosk number. I came home and submitted the rebate after inputting all that information.

anyone can share a light, what is the cheapest to port-out t-mobile and port-back in? i need to keep my permanent numbers. but don't need the FREE line # yet.
