Basic idea is to create your own certificate to sign ECG.tpk application and install it on your watch, then install the Samsung Health Monitor Mod app on your phone.
I have read lots of threads on XDA and can't find a complete tutorial about how to do it for Galaxy Watch 3 or Active 2. Hope this step-by-step tutorial will help. The whole process took me around 30 mins, it shouldn't be very hard. Make sure you back-up your watch first, just in case anything goes wrong and you need to restore data.
You will need an Android Phone, a Windows PC, and Galaxy Watch 3/Active 2 watch for this tutorial. And a wireless network.
Downloads:
Download Java for Windows offline (64 bit) https://www.java.com/en/download/manual.jsp"]https://www.java.com/en/download/manual.jsp and install it.
Download JavaSE/Oracle JDK https://www.oracle.com/technetwor...in
Download Tizen SDK https://developer.tizen
Download ecg.tpk, BP.tpk, Fit2Installer from https://share.cooltechs
Download Samsung Health Monitor Version 1..1.0.167.Caravana (Thanks Caravana for the Mod) from https://share.cooltechs
On Your Phone:
Install Samsung Health Monitor Version 1..1.0.167.Caravana on your phone. Open the app enter your name, BOD and etc to finish the setup.
On Your Watch:
From your watch, Go to "Settings"->"About Watch"->"Software"->Click On "Software Versio" 5 times until it shows "Developer mode turned on"
Go back to "About Watch", make sure it shows "Debugging is turned on", if not, click "Debugging" and turn it on.
Go to Wifi settings on the watch, change wifi setting to always On, and connect it to the same wireless network with your PC.
Reboot the watch. Make sure wifi is connected after the reboot.
From Your PC:
Launch Tizen Package Manager and Navigate to 5.5 wearable(It depends on your watch's software version, eg mine shows Tizen Version 5.5.0.1, so I navigate to 5.5 wearable), then click Install icon
Scroll down to the Tizen SDK tool and click the install icon
Click on the Extension SDK tab
Click the arrow on Extras
Install Samsung Certificate Extension and Samsung Wearable Extension
Check the Progress tab, once it shows 100%, you can close Tizen Package Manger and do not launch Tizen Studio(we don't need to use Tizen Studio)
Open up "Certificate Manager"
Click on the "+" icon and from the pop-up window click Samsung Logo.
Device Type -> Mobile/Wearable, Click Next
Certificate Profile(Give it whatever name you like), Click Next
Author Certificate -> Create a new author certificate, Click Next
Author Certificate -> Enter Author name, Password, Confirm Password as required(Remember the password,. you will need it when you sign the TPK file) and click next
A pop up will ask you to sign in to your Samsung account, enter your account information and sign in
Now it will show your certificate has been saved at C:/Users/USERNAME/SamsungCertificate/cert_name and click next.
Create a new distributor certificate and click next
Do not change Privilege and password information, do not close these windows, just leave it there, we need to enable debug mode from the watch and connect it to the PC now.
Open "Device Manager" from the PC.
Click the "Remote Device Manager" icon from the top right corner.
From pop-up windows, click the "Scan Devices" icon from the top right corner.
Once it finds your watch turn the connection switch to on. and close pop-up windows. At this time, you should able to see lots of debugging information from the Device Manager.
Go back to Tizen Certificate Manager, step 9.8, we can see one DUID has been added automatically. Click next.
It will show your certificate has been saved at C:/Users/USERNAME/SamsungCertificate/cert_name, Click Finish.
Download Fit2installer.zip unzip the whole folder to your desktop.
Go to the certificate(C:/Users/USERNAME/SamsungCertificate/cert_name) you just created and copy all files. inside the folder
Go to C:\Users\USERNAME\Desktop\Fit2Installer\cert Paste all the certificate file you just copied.
Copy ecg.tpk file and paste it at C:\Users\USERNAME\Desktop\Fit2Installer\sign_me
Click sign.bat from the Fit2Installer folder. Enter your certificate password.
It will show "Package( C:\Users\USERNAME\Desktop\Fit2Installer\install_me\ecg.tpk ) is created successfully." and enter any key to close the window.
Now go to "Device Manager", right-click your device, choose "install app", Select ecg.tpk file from C:\Users\USERNAME\Desktop\Fit2Installer\install_me.
Wait a few seconds, you should see ECG app has been installed on your watch. Open it.
Open the "Samsung Health Monitor" app from your phone, you should be able to read the ECG data from your watch now.
As Blood Pressure has not been released in the U.S, once it released, you should be able to activate for Non-Samsung phone the same way. You can disable debug mode if you want now.

For some reason I thought they were discontinuing it. I can't wait for Note 21 (if they actually make one, I heard they might nix Note line) Loving the Note 8 but I hated the curved screen, made it absolutely necessary to have case on it to be able to type long emails or basic usage without accidental presses.

What I don't like about the LTE version is it's one more device that is tracking me like a dog at all times. And that the battery life won't be as good.
I ride a bike so It's nice to know if I take a ride without my phone I can still call 911 if someone hits me, for example, and even take a short ride listening to MP3s, and have a map/gps in a pinch. Sometimes I want to leave at home my cell phone which is both a demanding universe that comes with the vague existential fear that the phone can break at any moment due to a fall, or dropping it.
Spotify works pretty darn well on these galaxy watches as well, though I will probably not pay for Spotify past my trial to be frugal, even though I love the service, especially the easy high quality downloading for offline listening on any device, including galaxy watches.
Such a nice deal. I'm assuming the galaxy 4 is about to come out? Fine with me. I'm sure it will be much more, and this galaxy 3 is fine for now. By the time the galaxy 3 isn't the galaxy 4, or 5 will be on a fire sale. Paying $250 for a watch for 2 years is plenty.
The galaxy Active 2 silicone wrist band is so nice (though I will be taking mine back to costco). I fall asleep with the Active 2 and wake up and my wrist feels fine, like I haven't been wearing a watch for 20 hours. I'm guessing the galaxy 3 wrist band isn't as comfortable. And guessing buying a Active 2 silcone wrist band and putting it on the galaxy 3 will make the galaxy 3 look weird. I'll have to figure out something there.

Why not carry a flip phone or something in a little bag by the bike seat in case you need to call 911. Any phone with or without SIM will call 911, and no tracking with those old phones.
Which Active2 do you have 41 or 44mm? I am not sure I can sleep with 44mm, I got it from costco, still sitting in box, contemplating returning it but I figured for about $180 after taxes, might be worth keeping it.
Funny you mentioned spotify, I cannot believe these things come with 4 or 8GB of storage. Storage costs so little and you get get GB's of space in a itsy bitsy tiny chips these days, 8GB (4GB on Active) sounds crazy.
Silicon bands are in fact very comfortable, but I've been considering one of those metal ones for the Active 2 if I start using it.
