For all approved rebates, please allow up to 14 weeks processing time to receive your Costco Shop Card.
If you qualify for this rebate your Costco Shop Card will be mailed to the address you provided during rebate redemption up to 14 weeks after submission. Other terms and conditions: Offer is only valid on promotion dates above. Rebate is only valid if handset is activated at Costco Warehouse.
All submissions must be redeemed online no later than the 30th day from the date you activated your qualifying service. Carrier service must be active at time of redemption to qualify. Costco Shop Card cannot be delivered to PO boxes. Return of qualifying device will result in forfeiture of Costco Shop Card offer.

My experience:
I activated a line there this afternoon on my grandfathered Simple Choice plan.
I called two Costco around me, one requires me to convert my plan to Magenta, the other one doesn't, so I naturally went to the latter. They also required me to actually bring the device over, than just bringing the IMEI#. I guess they want to make sure I'm actually the device owner.
I wasn't charged a SIM card fee or an activation fee. The whole process was very easy as my rep was really laid back.
He did have to call the T-mobile retail service line though, because somehow my account was mistakenly labeled as "FlexPay account", which didn't make sense to anyone but they were able to fix it. My rep had to provide some sort of authorization code to T-mobile over the phone though, I'm not sure what exactly.
After I got home, I realized that the new line I added had a $45/mo data plan on it, as T-mobile's system no longer offers our grandfathered data plan by default, so I did have to make another phone call to T-mobile to get that fixed.
Overall I think it was worth my time for $300.

I suggest that you go back to the kiosk and ask them to print out a copy of your incident ticket because that's the proof that they owe you the rebate.
To get them to submit the rebate, you'll need to do one of the following:
1) wait a week or two, and complain to the Costco store manager about your missing rebate
2) file a BBB complaint
3) call the Wireless Advocates customer service number and you might get lucky if they submit your rebate (only 1 person got this to work). Most of the time, they'll give you the runaround and tell you to talk to your kiosk guy.
If you did 2 lines, the sleazy Wireless Advocates rep might manually submit only 1 rebate. After waiting 11 weeks, you'll receive 1 out of the 2 rebates. You'll have to call them again and they might manually send you the 2nd rebate card. This happened to me and another person on SD. He received his 2nd rebate after calling but I'm still waiting for mine.
