Activate the offer in the Google Pay app.
You must be opted in to Google Pay Rewards and have a balance in your Google Pay balance card. This seems to mean you have to change to G Pay Visa debit card as your default payment method before making the transaction and have enough balance on it to pay for the transaction.
If you have a different credit card set up as the default, open the G Pay app, click on the credit card you are using at the top of the app, then slide over to G Pay Visa debit card (to the left of my Costco Visa in my app).
Minimum $5 spend per transaction.
Use your balance at qualifying US grocery stores and restaurants accepting Google Pay.
While supplies last.

Edit: I used this deal on 9/17. I had to change the payment method back to the G Pay Visa debit card manually before putting my phone close to the reader at the store. If you scroll down after choosing it, you can set it as the default payment method. The $3 cash back notification came within a minute of the transaction. You have to click on the gift box in the app to apply it to your account.
Update: A deal editor changed this deal from $0 to $2. The $2 reflects the minimum net amount you have to spend on each transaction to get each reward. That is, if you do three $5 transactions for $15 total, your net cost is $6 after the $9 cash back. After a poster below said the offer was no longer valid, I redeemed the offer on my partner's phone. A member of staff was able to redeem it too, see post #6.

Once it's installed and open, click the tag icon in the bottom left of the home screen of the app. Scroll down a little to the section Earn with Google Pay and add the offer, if you have it.
If it still isn't there I'll change the title to YMMV.
I found the offer in Google Pay app but when I clicked View Details a popup stated "This offer is no longer available"

That actually says SSN instead of last 4 digits of SSN. Yikes!
Not a huge deal. But it's not a ton of efforts either.
