You can purchase compact heads for smaller mouths. The toothbrush works with both.
They have two different sizes available for kids brush heads.
I have this tooth brush for both of my kids and they're very good.
I use smaller size brush head for 6 years old and bigger for 9 years old.

Do know that generic brush heads are always a crap shoot. They range from good (none of them equal original quality but many are definitely close enough) to terrible.
