I guess I wasted $1 on this deal, as I have no use for this until May. It looks like the Boost Mobile rep gave some very wrong info at the start of this thread.

Our team has been working to figure out what has occurred with this receipt and I now have some answers to give y'all. Please rest assured that you are NOT being charged a $50 activation fee for this promotion, you are only paying $0.99 + any tax depending on your area. We are working to now update these receipts to accurately show what you are all paying for today.

This should work with a Visible port in, but you do not have to port in to get the deal. You can contact Boost Mobile Customer service direct at (1-833-502-6678) to verify or you can private message me and I can help further there.
Is there a date by which we need to activate or lose the promo rate?
With our new feature being introduced in mid-December, Express Activation. Your SIM card will be automatically activated 2-3 days after shipping.
This new feature makes it easier for customers to activate as soon as it arrives.
At checkout you will see a choice under "Express Activation" (Which is required) to choose between porting in your existing number or having a new number assigned to you. Here you will also find an explanation about Express Activation and Autopay.
