It worked fine for me. I see a lot of people saying it didn't work, so I'd be curious to know why only "select" customers get the offer and what makes someone a "selected" customer.

For Amazon card and other Chase card holders, every quarter check this link for the latest promos. Sometimes you get a hit (Amazon card), and sometimes you don't, (Priority Club).
No go on my Chase Amazon Visa card (dark gray metal card).
Edit: It worked the 2nd time I tried. I'm 100% certain I input the exact SAME information because it was already prefilled for me from the first attempt. Weird.
If it didn't work for you the first time, try again. It's worth a shot.

Always try these kind of things a couple/3 times. I've found that multiple tries often does the trick…
