you should have been faster at using the promo or like others, wait back until a few orders are cancelled and the promo becomes available again
You should have seen that the promo was done before the shopper was assigned.
Right after the checkout screen and all the tip/download the app/add more items nonsense, that's when you see the real total.
I've noticed during that window there are quite a few things to be careful of after the order has gone through. You an actually remove items before a shopper is assigned and if the total hits below $100 it might show the promo going through but in the end it recalculates after the shopper is finished and it does not apply
This coupon is totally screwed up. It has other issues too. Anyone using it is doing so at a big risk (to their time).
A safer alternative is to deliberately order out of stock items. This one works pretty damn well only risk is for grocery stores with multiple locations, instacart does send shoppers to other stores sometimes. You can't guarantee where they shop at, so stores with few locations like costco it is safe

If the shopper refunds an item that is out of stock there is no affect on any discount or promo. I believe also buying items that have a coupon attached do not allow you to use a promo.
So any instant savings is not allowed to be combined with promos.

I'll tell you this, we slightly modified a $140 order, by adding a $10 item, and removing a $15 item, in that order, and we lost the coupon. We don't have any items with in-store coupons, or excluded items. It cost us 2 hours in screwing around with Instacart CS, and they were clueless. I figured a way around it, but that doesn't mean we won't get screwed when they try to shop it.
Whomever programmed this coupon really messed up. CS did confirm they are getting a ton of calls about it.

Imagine, if just adding a bag of potatoes to an existing grocery order voided these coupons?!! Well, it did for us - no joke. It's one reason why I think so many coupons are being lost. Don't edit your carts, folks, or you might end up screwed.

First time using Instacart and was really surprise I can order heavy stuff and have them delivered within a couple of hours. Lowes would have charged me $80something to have these bags deliver tomorrow, not within a couple of hours.
I'm sold using Instacart now knowing I can order anything heavy up to 50lbs to deliver to my door.
