CS rep indicated that the offer comes and goes. Check any texts you may have received from Verizon to see if you received it. I don't know the requirements for it to be triggered, but it may be due to the fact that I was actively searching for alternative carriers. I was googling 'mint mobile' and 'visible' on my phone. I have myVerizon app installed as well, and it does track the websites you go to (check in app privacy report for iOS users).
In order for it to kick in, and you've been a Verizon subscriber for awhile, try searching for alternative mobile carriers on your phone (make sure you have myVerizon app installed before searching) and see if you get a text message from Verizon.
