Called Costco to verify the deal. They have been having this deal for some time and this current one expires on 10/26 but they think it will be extended. They only have black iPhone 14 in stock at my location. No rate plan restriction for upgrade.

up to $360 credits on your trade-in.

Mail in should work within 30 days. Ive done Costco trade-in previously and assuming it's the same.

Usually you need to have the iPhone that is being traded in so they can write down the imei and check to see if it's cracked and for water damage and iCloud lock

They print out a sheet which includes a prepaid label. You ship it out on your own. I'd suggest putting it in a bubble padded mailer and then putting that mailer in a box
