look at 3 attachments from when i did this deal a few months back.
in bay area , ca
when i had only 1 line.===>total was 16.44
when i had 5 lines===> total was 21.75
in middle when i added 2 lines on 8/24 and 2 lines on 8/26.
the bill as crazyhigh.
and this was after getting a 60 courtesy credit !!

Dont have multiple phones with me but I want to buy 2 numbers and will activate in a single phone .
So use 2 IMEI when ordering and use 1 IMEI for activating it.

I did it 3/4 on they Visa card and I need to call them multiple time to get it even I did it everything correctly.
