Update: Bostonyankee's suggestion about account managers (on page 6 of this discussion) worked for me.
My 12 month required service with Verizon(for previous gift card promotion) is ending soon. Probably won't stay with verizon just for this promotion. But if you're with verizon anyway, this seems to be a great deal.

1. Try going into Account Management when logged in as the Account Owner (the login that doesn't let the promotion go through).
2. Add an Account Manager and associate them with one of the remaining phone numbers on the account. If you already have an Account Manager set up like this (with # and email), just use that login in step 4.
3. Sign out of the Verizon site
4. Sign in as the Account Manager, not the Account Owner.
5. While signed in as the Account Manager, go to plusplay.verizon.com and try the steps to activate the offer you want.
6. Profit!
Hope that works for others.
Bad news: since this is Verizon, they found a way to screw everything up. After I completed sign-up, etc., I got an email telling me the payment was declined, and everything was cancelled. Extra funny: the credit card I used was the Verizon Credit Card. When I called Verizon, they told me the bank wants me to call them and confirm the charge, but there's no way to undo the cancellation. There's also no way to reapply the promotion, since it now has been used.
So far, Netflix still thinks it's on Verizon, but I'm sure that's a temporary state of affairs and it will eventually realize that everything is screwed up. I doubt I'll ever get Verizon to fix it.
