Also best to call local costco to confirm t-mobile at kiosks before heading there.
t-mobile costco kiosks are staffed by actual t-mobile employees from nearby stores. Some costco kiosks are staffed by AT&T, some Verizon, some kiosks are closed.
Offer is good even for t-mobile grandfathered plan when u upgrade or buy device.

Most of t-mobile promotions eligible on t-mobile stores, can also be done at costco t-mobile kiosks, with additional benefit above.

some costco location have AT&T and some have T-mobile

if u bring your own device, u must be eligible to add a line on your grandfathered (some grandfathered account will not allow AAL). If u can add, then ur eligible per my understanding, though I'm not related to t-mobile, except being a satisfied customer. Keep us updated Smilie

Don't forget $200 port in credit as well, if coming from some big carriers like ATT, Verizon, Xfinity.
