I had no issues getting it done with no appointment. 15 mins and got the card expedited for 2 day delivery.

If you're married you can always have spouse sign up for it and then get yourself added as an authorized user and get a duplicate card to use.
You could try calling before, but I went today and just brought up the image from the OP's link. No issues except that I needed to make an appointment but it was same day 2 hours later. Process took less than 30 minutes.
