1 - they thought they were ordering directly from Amazon, but instead it was a 3rd party on behalf of Amazon. A lot of them have "PRIME" so it can easily get confusing .
2 - the worker pulled from the wrong bin. I feel that's much more uncommon.
This has never happened to me in the 20+ years of using Amazon, but I have noticed many times over the past few years the ease that you can see a deal that qualifies for Prime but it's not directly "shipped" by Amazon but instead a 3rd party....and if you click on the "other listers" or whatnots you'd see Amazon but maybe it won't ship for another week or whatnots vs that 3rd party.

Review on that item said they received a PS5 in bubble wrap when it was listed as new.
Also this bundle seems to be 10 dollars more than their website, but on both pages it says "Bundle games codes may be used" not sure why it would say that on brand new.
