bizarre ..? i agree. ...And usually unheard of.
but has happened
1. Looked up Costco store locator with T-Mobile in-store kiosk and went there
2. Requested upgrade for 3 lines with the Motorola 2023 Edge promotion (kiosk had none in stock)
3. Customer rep knew about it and requested my trade-ins and lines to upgrade
4. Gave rep 3 burner phones locked to Sprint that were in my drawer since 2015 (rep didn't test any of them)
5. Upgraded each of 3 lines using Only2023MotorolaEdge option or something like that, checked IMEIs on trade-ins, all clean (none blacklisted)
6. Waived activation fees for all 3 lines, only paid taxes based on shipping address for new phones
7. Was advised that $75 Costco Shop Card eligibility is confirmed because transaction was done at Costco T-Mobile kiosk and verified by shipping address matching that of Costco member in household
8. Was emailed Docusign EIP agreement, which I signed to initiate shipping process
9. Rep gave back trade-ins (marking each to the line that was upgraded) and advised me to drop them off at a local T-Mobile store when new phones were delivered
10. Was in and out within 30 min.
Edit: No changes were made to my rate plan as far as I can see or was told by the rep.
So now, just waiting on delivery to finish the trade-in process, activate the phones and watch for the bill credits to kick-in and Shop cards sent to me when available.
If you just take a picture of the steps I've outlined and visit a Costco with the T-Mobile kiosk in-store, you should be able to replicate this fairly easily.

The phone line that the Moto phone is associated with (on an EIP plan with 24 monthly credits) must be active for the whole 24 months (so the Moto phone is free). You may use any phone on that phone line. You may use the Moto phone with any T Mobile network line or even not use it and keep it in the sealed box. If you cancel that phone line anytime during this 24-month period, you will be charged for the Moto phone for any remaining credit amount.

Rep said they'll just check the IMEIs to ensure they all match the line that was upgraded, then throw into a shipping box to wherever.
The promo did say any phone, any condition so it doesn't seem like there should be any real issue, but I suppose we'll find out soon enough!

In the past though, I read many rejections. That's why I mentioned it.

Traded in an old Nokia 635 windows phone Att version. I believe it was from an old slickdeals deal..go phone I believe. And only used it for a month, so no doubt it was still locked.
The rep checked imei said everything looks ok, put me on hold while she had someone authorize waiving the activation charge, which I didn't even have to ask for.
Emailed me doc to sign, paid just the tax, told me I can send trade in in or take to Tmbob store. Said 75 shop card will arrive in 3 to 4 months. They wait 90 days before processing the card.
And that was it. Took about 35 minutes over phone. No hiccups

Found an old nexus 5 in my drawer this morning. So called back. Got a rep immediately (amazingly the same rep as on Thursday). Also took about 20
Mins. I was a little concerned about this one. I got it off eBay yeats ago and used it with att as the imei was black listed on T-Mobile. However he checked the imei and said it was cool.
So with that I am in for 3. Out of pocket $127 for tax. $225 in gift card on the way. Thinking I may keep 1 phone as a spare and flip the other 2 on eBay. Seem to be worth about $250.

no where i see a mention of costco $75 cash card. Is that fine? they also did not take my device asked me to go to tmobile store to drop by or print shipping label and send it withint 30 days.
