our experience as follow:
- went to the kiosk
- mentioned directly we wanted to upgrade using promo P444
- the staff offered some monthly protection, I think it was $10/month or something. I declined without any follow up from them
- at first, she mumbling something like, the system wouldn't take the trade-in, but they kept trying
- finally, it went through just fine, and around 20-30 minutes later, got text confirmation that the trade in was accepted at $0, and asked me to return the trade-in device to the store
the good:
- I didn't have to ask the staff to waive DCC. they did it right away
- got 2 day shipment free
- I could return the trade in device to the store
the bad/questionable:
- they took a picture of my driver license, as verification
(I FELT VERY UNEASY ABOUT THIS)
- they seemed to rush us. mind you that no one was waiting. so we didn't get a chance to confirm if the promo code was placed properly
- the staff insisted that one trade PER LINE. I used to be able to get TWO EIPs/trades PER LINE.
- the staff did not explain how they tie my COSTCO to T-Mobile, like at all.
I will check my account tomorrow to see if the transactions were done properly.
one more thing, the staff mentioned to let them know if there's issue, instead calling support.
kinda fishy, but they said the corporate would reduce their performance score if I call support for whatever reason.

forgot to mention: the TMobile staff told me they didn't carry many stock in the store, but still plenty available for shipping
another update: I just found out that motorola actually sells this model for $299, with 256gb storage. while TM sells the 128gb version for $498.
If you're going through the Costco route, it's safer to do this in store with a TMO associate in person than on the phone.
I tried to do the upgrade by calling Costco's T-Mobile number and now the phone with a broken screen I planned to trade-in can never be traded in through T-Mobile.
I logged on Tmobile's website and tested the promo using a Lumia 640 as the trade in. All the trade-in info, device info, and promo looked right. Then I called the Costco T-Mo hotline to do the upgrade and the associate was full of crap: I wanted to trade in a Lumia 640, he said it had to be unlocked (it is). I gave him the IMEI, but he can't verify it.
Then he said I can only trade in my current phone (a blue iPhone 12) for P444. I told him I won't. He said that's the only phone I can trade in and he can't process any other device. I asked him to point out the terms where that's the case. Then he said I had to have used the device for 24 months before trading it in through upgrading, and so that's my iPhone and nothing else. I told him he's wrong and I'll try in-store. Where do they find these lying associates anyway?
After that call, now when I input in my Lumia's IMEI on the trade-in page, both in-store and online, it autofills the device info as "iPhone 12 Blue", same as my current phone. So if I trade it in, they're expecting an iPhone 12 for this Lumia's IMEI. I don't know what the rep on the phone did or how to fix it.
If using T-mobile's postpaid bring-your-own-device IMEI checker, it comes out as the correct device: "Microsoft Lumia 640," but not on the trade in page and I can't select any different make/model after inputting the IMEI. When I put my sim into the Lumia, the Lumia's IMEI now registers as a blue iPhone 12 on my account page even after a few days. Swappa's IMEI checker also shows the correct Lumia 640 info.
Don't call.
and boo, Tmobile, for stopping the autopay $5/line discount if I don't use a debit card/bank ACH. They lost my personal info through breaches multiple times.
