Your prepaid card will be sent to the mailing address or email address submitted on your online redemption form. You must maintain your qualifying services and keep your account active and in good standing (no past-due balances) for a minimum of 90 days and through the date your redemption request is processed.

Also, this part of the promo "and through the date your redemption request is processed", gives me pause. Does this mean I have to wait until I know my $500 redemption has been processed which could be longer than the 90 days?

If you order the phone from Apple now, you won't get it until mid Oct. so it really just an extra month delay.
