Yes, the $400 is on top of the trade in. As mentioned, there is a Costco member credit and port credit. For the port credit, you need to port from the other 2 carriers, at least on their network for 90 days, and not a Tmo MVNO.
I only see the $250 but there's no mention of the additional $150 Costco card. Those of you who received the additional Costco cards, did you need to submit any additional promo requests to get it or is it automatic with the one I did above?
