Amazon offers a $4 credit to be applied towards purchase of Tempations Creamy puree cat treats when you complete the short quiz.

Same here. Tried it again on my own & was offered a $3, $4, $8 coupons on the 16, 24, 48 counts (chicken) - without any quiz.
uninstall app, use browser select quiz, then add the product.
